It's easy to participate in CAIR and there's no cost to enroll or use the system. To enroll in CAIR, you must be a healthcare provider, school, childcare facility, WIC facility, or Foster Care agency.
To sign-up, complete the 2 forms below and FAX to 1-888-436-8320:
If you have questions, contact the CAIR Help Desk team (1-800-578-7889). To add a new facility to to an existing provider account, submit the CAIR Provider/Agency Access Agreement form (above). Be sure to enter your Provider Group ID. If you are not sure what your Provider Group ID is, call the CAIR Help Desk. To add new users to your facility, use the CAIR User Access Agreement form (above).
What's Happens Next?
- Once your account has been created, you will be contacted by the CAIR Help Desk or your local CAIR representative to discuss training options.

- Familiarize yourself with the patient/parent diclosure letter (all patients/parents must read this before their immunization records can be added to CAIR)
